On Sep. 29, PHI BWC issued a report for completion and submission.
PHI BWC issued report titled Annual work accident or illness exposure data report.
This shall be accomplished whether or not there were accident/illness occurrences during period covered and submitted to Regional Labor Office or local government having jurisdiction not later than 30th day of month after end of each calendar year.
Report Fields
Report includes fields for name of establishment; nature of business; and address.
Plus, fields on exposure data like number of employees, total hours worked; injury summary i.e. total-all disabling injuries/illnesses; frequency rate; and severity rate.
The report is to be signed by the general manager of the establishment in question.
Effectiveness
According to the report, the next reporting deadline is Jan. 30, 2025.
