Breast Exam Coverage
NH INS 417-D:2-c added so that no group plan, or issuer offering group or individual health insurance can impose cost sharing requirements for the specified services.
Requirement applies for high deductible plans only if minimum deductible has been met, except for items and services that deal with preventative care as per 26 USC 223.
Defined cost-sharing requirements, diagnostic and supplemental breast examination.
