2026 Issuer Guidance
Reflects guidance set forth in proposed Notice of Benefit and Payment Parameters for 2026 (NBPP) and draft 2026 Letter to Issuers in the federally-facilitated exchanges.

Qualified Health Plan (QHP) Guidance
NH INS published the CMS updated maximum annual limits on cost sharing.
Must file report showing compliance with prescription drug rebates by Mar. 1 each year.
The mental health parity quantitative treatment limits (QTL) reporting tool must be completed for each QHP plan and submitted through the SERFF by May 1, 2025.
Issuers shall complete and submit the network adequacy template for each QHP filing.
Essential health benefit category and class drug count tool, the adverse tiering tool, and the non-discrimination clinical appropriateness tool are required for issuers to run.
