  • A health benefit plan shall accept a correctly charged mileage rate as an allowable expense for a claim, including acceptance of claims for out-of-network providers.
  • Health benefit plan shall accept a correctly charged amount equal to base rate as an allowable expense, or equal to the medication maximum cost as an allowable expense.
  • Claim provisions also apply to an employer's workers' compensation insurance carriers.
  • Defines balance bill as practice of a health care provider billing an individual for the difference between individual's billed charges and the amount covered by health plan.
  • Base rate is the rate ground ambulance provider charges for transporting an individual to hospital, supplies used during transport, medication or procedures during transport.
  • Bureau shall establish a maximum mileage rate for ground ambulance providers.
