Requires health coverage for outpatient use of pasteurized donor human milk (PDHM).
For infants who are medically or physically unable to receive maternal breast milk.
Or if mother is medically or physically unable to produce maternal breast milk at all or in sufficient quantities or facilitate breast feeding despite optimal lactation support.
Applies to individual, small group, large group, and article 43 health insurance plans.
