Provisions applicable to large and small group health care service plan contracts and disability insurance policies issued, amended, or renewed on or after Jul. 1, 2025.
Required to provide coverage for diagnosis, treatment of infertility, fertility services.
With respect to large group health care service plan contracts and disability insurance policies, required coverage for maximum of 3 completed oocyte retrievals, as specified.
Revised infertility definition, removed exclusion of in vitro fertilization from coverage.
