CAN ONT FSRA revised Application for Accident Benefits (OCF-1) form to improve access to automobile accident benefits, to make it easier to understand and complete.
FSRA is allowing a six-month transition where insurers can accept both form versions.
Effectiveness
As of May 7, 2025, only the updated form will be available for filing accident benefits.
