Online Submission
All HMOs must submit their AFS and attachments through IC online uploading portal.
Documents must be arranged numerically in sub-folders according to item numbers indicated in annex A, all items in checklist must be submitted in one compressed, password-protected folder, formats, headings, footnote instructions strictly followed.
Effectiveness
Submission deadline is 11:59 PM, May 31 annually, moving to the next working day if May 31 falls on a weekend or holiday; incomplete submissions will not be accepted, and company will be notified about missing items; PHI INS will issue order of payment.
The circular takes effect immediately as of Mar. 31, 2025.
