Each carrier that files a drug formulary shall also provide the policies and procedures that govern its P&T Committee with respect to the management of the formulary.
Particular emphasis placed on the P&T Committee's membership requirements, conflict of interest prohibitions, drug distribution process, and anti-discrimination policy.
Survey Deadline
Survey shall be submitted by Oct. 1, 2024 and no later than Oct. 1 annually thereafter.
